Human Services Positions Last Updated: July 16, 2008
QMRP- Habilitation Department Responsible for the development and implementation of Habilitation Plans in accordance with the ISP. Serves as QMRP for assigned individuals, assisting program participants in the development of programs and ensuring the implementation of Habilitation Services. Maintains open lines of communication with Habilitation Instructors, Habilitation Assistants, Service Coordinators, and assigned Program Coordinator. Conducts training of staff at assigned programs in regards to the Habilitation Plans, service documentation, and safeguards. Validates billing monthly by ensuring all billing documents are in accordance with regulations, policy, and billing requirements. Ensures Hab Plan and all related documents are filed appropriately to ensure confidentiality as well as efficient filing. Provides QMRP review of all monthly summaries. Ensures the summary is in accordance with the plan and meets regulatory requirements. Bachelor’s Degree in Human Services or related field and one year of experience with OMRDD services.
Placement Specialist Acts as an Agency contact for individuals inquiring about residential services. Maintains databases and wait lists. Works with Service Coordinators to obtain names of potential candidates for residential services. Completes outreach activities. Contacts and meets with individuals on the waitlist, their family/advocate(s), and Service Coordinator to determine appropriate services. Conducts initial interviews and completes paperwork. Arranges tours of prospective sites for the individual,family and Service Coordinator. Schedules admissions with individuals moving into residential setting and completes admissions paperwork. Develops an initial Plan of Protective Oversight and Residential Habilitation Plan for individuals served upon admission. Attends and participates ISP meetings for consumers at the shelters. Completes Incident Reports and follow-up. Completes or assists with Utilization Reviews. Gathers and maintains applicable information pertaining to local, state and federal funding sources and/or oversight agencies, and forwards information to the Shelter Services Coordinator for review and preparation of necessary reports. Bachelor’s Degree required. Use of personal vehicle required. Mileage reimbursement provided.
Day Hab Instructor Provides site-based skills development training as well as direct assistance and support to individuals with developmental disabilities. Conducts assessments and develops day habilitation plans that are person-centered. Documents services in consumer case record. Transports or provides mobility training to individuals being served in the program. 27 - 30 hours per week. Clean and valid NYSDL and High School Diploma/GED required.
Budgeting Aide Provides training, instruction and assistance in money management and budgeting to individuals with developmental disabilities. Completes individualized budget and budgeting sheet and reviews income and expenditures with consumer. Reviews bills and ensures prompt payment of rent and utility bills. Monitors progress in instructional activities and informs Service Coordinator of level of progress monthly. Transports consumers for budget related activities. Must possess good communication skills, working knowledge of DD population, math computation skills, and be able to balance a checkbook. Clean and valid NYSDL and High School Diploma/GED required. Use of personal vehicle required. Mileage reimbursement provided.
Life Skills Trainers and Residential Relief Provide support and supervision to individuals with developmental disabilities who reside at Community Services Residential Programs. Teach individuals life skills to encourage participation in the community. Assist with personal care, cooking, cleaning and other daily living skills as needed. Provide counseling and crisis intervention as needed. Dispense medications to individuals as required. Coordinate with the pharmacy, case managers, and doctors to ensure that adequate supplies of medications are on hand. All shifts available. Clean and valid NYSDL and High School Diploma/GED required.
Lead Life Skills Trainer Provides support and supervision to individuals with developmental disabilities within Community Services Residential Programs. Assumes the lead role on shift in the absence of the Site Manager. Provides modeling and guidance to site staff to ensure completion of shift responsibilities as well as ensure accurate communication of events, issues and daily outcomes. Completes environmental reviews of the site and vehicle. Assists with personal care, cooking, cleaning and other daily living skills as needed. Conducts fire drills as assigned by the Site Manager. Collects receipts for operating expenses and prepares reimbursement forms. Assists Site Manager in monitoring staffing coverage for site and identifying potential candidates to fill open shifts or to cover sick calls and emergency staffing needs. Ensures potential incidents are identified and reported in a timely fashion. Ensures incident reports are completed and appropriate notifications are made. Knowledge of developmental disabilities required. Must have good oral and written communication skills. Must be able to read and write consumer case notes as well as understand written direction. Must have High School Diploma/GED and one year experience as well as clean and valid NY Driver's License.
Hab Instructor Full time, Part time and Relief opportunities to help individuals with developmental disabilities maintain their independence while supporting their personal growth. While base out of Buffalo office, position involves primarilly Southtowns caseload. Position provided one-on-one and small group instruction to individuals who are developmentally disabled and their families. Instruction provided in the areas of budgeting, nutrition, personal health and wellness, mobility training, leisure, cooking, household management and other daily living skills. Position requires the ability to read and write casenotes and problem solve. Time mangement skills and good work ethic are essential for success in the Hab Instructor position. High School Diploma / GED required. Must have use of personal vehicle and be willing to transport individuals served in your personal vehicle. Mileage reimbursement provided. Hotline Support Staff 10 - 15 hours per week. Provides information, referrals, counseling and intervention to individuals with developmental disabilities and their families who utilize the support hotline. Provides outreach and assurance to individuals served and their families upon request. High School Diploma or GED required. Knowledge and/or experience with developmental disabilities preferred. Hours of operation are 4:30pm-9:30pm Monday – Friday and noon – 5pm Saturday and Sunday.
Job Coach Relief position- Evaluates, trains, and supervises assigned individuals in the Supported Employment Program. Conducts assessment of assigned individuals to identify vocational skills, barriers and preferences. Assists individuals served in developing resumes, employment applications, job interviews, and other hiring requirements. Teaches job skill development and intensive job training. Provides individualized mobility training to and from the individual’s place of employment. Knowledge of how to maintain/operate maintenance equipment and of developmental disabilities required. High School Diploma or GED needed. Must have a clean and valid NYSDL and use of personal vehicle. Mileage reimbursement provided.
Administrative and Management Positions Last Updated: July 16, 2008
Clerk Typist - Human Resources Types memos, letters, forms, and other documents for members of Administrative Services. Files applications, documents, and forms. Completes faxing projects. Maintains and updates applicant database. Assists in obtaining reference and background checks for all applicants eligible for hire. Maintains all department forms and manuals and completes additional duties as assigned. Requires confidential use of employee related information. High School Diploma or GED required. 20 hours per week.
Director of Finance Responsibilities include supervising Accounts Receivable, Accounts Payable and Purchasing, preparing reports required by multiple funding sources and maintaining general ledger and fixed assets records. Provides oversight of payroll (ADP software) and management of client funds. Successful candidate will be organized, analytical and detail oriented with experience developing and maintaining internal controls. Requirements include exemplary accounting skills, computer competency, and either a relevant Master’s Degree with two years of experience or a relevant Bachelor’s Degree with four years of experience. Two years of supervisory experience also required. Knowledge of OMRDD rules and regulations and CFR reporting experience preferred.
Site Manager Oversees hiring, training, scheduling and supervision of staff. Oversees management of petty cash expenditures and PMF accounts. Responsible for monitoring and implementing Residential Habilitation Plans. Ensures assignment, completion and documentation of in service training of IRA staff. Manages expenditures of Petty Cash, Food Stamps and the individuals’ personal funds. Ensures protection and care of individuals served through review of Incident Reports. Ensures compliance with all OMRDD certification requirements. Ensures routine maintenance and repairs of the Site and the Site vehicle. Provides rotational 24 hour on call support to IRA staff.
Senior Staffing Specialist Leads, directs and advices customer service driven team in the pursuit of ideal candidates for growing agency. Proactively meets the needs of the programs served by finding new sources and building community relationships to promote and quickly fill all open positions. Leads diversity initiative and be responsible for completing staffing reports. Associate's Degree and 1 year of recruiting experience required. Community Services
