Service Coordination - By offering one-on-one support, service coordinators help develop goals, address needs and explore options.  They provide people with a developmental disability access to financial, medical, and habilitation resources.  The ultimate goal of every service coordinator is to help each individual obtain the supports necessary to reach his or her highest level of independence possible.  For more information, call our Admissions Department at 883-8002.

Programs for Families and Children - Service Coordinators offer support to parents and children with developmental disabilities.  They specialize in meeting the unique needs of families with a member with a developmental disability.  Assistance can be provided with obtaining or maintaining public benefits like Medicaid, Temporary Assistance, SSI and SSD; linking to outside services, including habilitation services, residential services, day programs, respite, and counseling; and providing advocacy during Family Court and CSE meetings. 

Children that are 3 years old or older and reside in a biological, foster or adoptive home are eligible for services at Community Services for the Developmentally Disabled. For more information call 883-8002 ext. 165.

Senior Services - The Senior Services Program was developed to meet the unique needs of individuals age 50 years and older.  The program places an emphasis on meeting medical needs and ensuring delivery of senior benefits.  Service Coordinators can provide linkage to senior day programs, retirement programs, housing and medical services. Through its partnership with the Erie County Probation Department, the Developmentally Disabled Offender Program is able to provide effective alternatives to incarceration for program participants. Programs are also available for children and young adults ages 12-25 who are at high risk of becoming involved with either substance abuse or the criminal justice system. Assertive Community Treatment (ACT) - The ACT Program is specially designed for individuals with a dual diagnosis – people with a developmental disability and a mental health diagnosis or a history of challenging behaviors.  Each person in the program has access to a Team Leader, Service Coordinator, Habilitation Worker, Registered Nurse, Behavior Clinician, and a Psychiatrist.  Services are provided entirely in the community and at the person's residence.  Individuals can access the Team 24 hours a day, 7 days a week through an on-call phone.  The ACT Program has had great success in providing services to people who have been unable or unwilling to remain in other programs. The Social Services program provides service coordination, linkage and referrals to individuals with specialized needs including personal or financial issues, medical concerns and employment needs. For more information call 883-8002, ext. 141.

Specialized Case Management (SCMP) - This is the only program of its kind in Western New York.  Specialized Case Management provides services to youth and adults with developmental disabilities that are considered at-risk or who are involved in the Criminal Justice System.  Service Coordinators will develop a plan based on individual needs to prevent recidivism; provide linkage to outside services including counseling and substance abuse treatment; provide advocacy and assistance throughout the court process and monitor legal obligations; and work with Probation Officers and Courts to establish a plan for success.  Specialized Case Management is recognized as an Alternative to incarceration program through DPCA.  For more information, call 883-8002 ext. 142.

Project COACH - Project COACH is an after-hours telephone support hotline for individuals with a developmental disability and their families.  COACH staff can provide information, referrals, outreach, counseling and crisis intervention.  They can also assist with medication reminders, mentor on telephone social skills, and do wellness check calls to prevent isolation and loneliness.  For more information, call 883-8002 ext. 139.
